This article aims to help the clinician prioritize the ocular trauma patient. The patient should be assessed for signs which indicate the necessity for urgent referral. The eye may be difficult to assess, particularly in children. Examination of the eye may be unfamiliar to the clinician and if there is any doubt the patient should be referred for ophthalmic assessment. Key features of each of the aspects of the ophthalmic examination are highlighted and advice provided about those patients which require urgent referral. Emphasis is placed on the acutely compromised optic nerve and perforated globe requiring urgent referral.

Key Words: eye • trauma • ocular perforation • relative afferent pupillary defect
